The translator prepares an evaluation and signs it in front of a notary. The notary notarizes the signature. Most banks have notaries available.
The translation should be written on a letterhead and at the top or bottom of the translation the translator types:

I reside at [address]. My phone number is [phone #] and my email address is [email address].
I hereby swear and affirm that following:
I am fluent in English and [other language] and I am not related to the person whose documents I have translated.
